CenterWell Referral Process (AOR / Agent of Record Link)

To refer a patient to CenterWell and qualify for support, giveaways, and financial assistance, agents must follow the process below. The AOR (Agent of Record) link must not be sent before completing these steps.

Step 1. Initial Message to CenterWell

Before sending any link, the agent must send a text message to Brittany, the CenterWell manager.

The message must include:

• A greeting

• Agent name

• Team name

• Notice that you are sending a patient

Example:
“Good afternoon, Brittany. I hope you are well. This is Daniel from the Golden Plan Choice team. I would like to send you a patient.”

Step 2. Send Patient Information

After the initial message, send the following patient information to Brittany:

• Patient full name

• Date of birth

• Phone number

Step 3. Wait for Confirmation

Brittany must confirm that the patient is already entered in the CenterWell system.
Do not send the AOR link before this confirmation.

Step 4. Send the AOR Link and Schedule the Appointment

Once confirmation is received:

• Use the AOR link to schedule the appointment

• Take a screenshot or photo of the appointment confirmation

• Send the confirmation image to Brittany

Step 5. Confirm the Clinic Location

Send Brittany the name of the CenterWell clinic the patient will attend.

Only referrals that follow this exact process allow CenterWell to provide support, financial assistance, giveaways, and other resources.
